What is the ICP Program at CBH?

The ICP Program at CBH is a Pay-for-Performance (P4P) program that promotes the Pennsylvania Department of Human Services (PA DHS) goal of greater integration and coordination of behavioral health and physical health services. The ICP program aims to improve the quality of health care and reduce expenditures through enhanced care coordination between PH-MCOs, BH-MCOs, Primary Contractors, and providers.

This program focuses on adult members with both physical and behavioral health needs based on criteria defined by PA DHS. Using this framework, CBH works with providers to address these members’ needs effectively.

What is an Integrated Care Plan?

An Integrated Care Plan (ICP) is a care coordination tool that consolidates an individual’s physical and behavioral health information into a single, comprehensive document.     Developed collaboratively by the individual’s physical and behavioral health insurance carriers and treatment providers, an ICP simplifies access to essential health information, supporting a more holistic approach to care.

An individual’s ICP may contain the following information:

  • Their physical and behavioral health history, including diagnoses, utilized services, prescribed medications, and treatment goals.
  • Contact information for each member of their care team, including physicians, therapists, case managers, and social workers.
  • Social determinants of health (SDOH) needs, including housing, transportation, employment, safety, food, childcare, etc.
  • Next steps the care team can take to assist the member in their treatment plan.
Who is eligible for an ICP?

To be eligible for an ICP, an individual must be:

  • An adult aged 18 – 64
  • Medicaid-eligible (excluding Medicare)
  • Diagnosed with a Serious and Persistent Mental Illness (SPMI) such as:
    • F20 – Schizophrenia Disorders.
    • F25 – Schizoaffective Disorders.
    • F28 & F29 – Other Non-organic Psychoses.
    • F30 – F33.9 – Episodic Mood Disorders.
    • F34 – Persistent Depressive Disorder.
    • F39 – Unspecified Mood Disorder.
    • F60.3 – Borderline Personality Disorder.
