CBH Provider Forum: Transitions of Care

The Transitions of Care Provider Virtual Forum is a CBH-hosted event designed for Acute Inpatient (AIP) and Outpatient (OP) behavioral health providers.
The event will focus on strengthening collaboration across levels of care to improve transitions and continuity of care for CBH members. Attendees will receive updates on key quality measures, review CBH interventions, and participate in breakout discussions tailored to adult and children’s services.
