CBH Provider Forum: Transition of Care

This free event is open to all CBH providers who offer Acute Inpatient Psychiatric (AIP) Services and Outpatient (OP) Mental Health Services and are involved in care transitions. Attendees will have the opportunity to collaborate with CBH and peers to address barriers related to members’ care transitions, discuss best practices in transitions of care, and learn about available tools in the network for enhancing care transitions.

• Barriers to medication access
• Connecting members to peer services
• Transportation services (Modivcare)
• HealthShare Exchange (HSX)
