Building Bridges to Better Health

CBH is committed to developing programs that address the complex behavioral health needs of people of all ages, backgrounds, and socio-economic levels. While barriers to care exist, CBH serves as the bridge to connect and support individuals through coordinated, person-centered services that strengthen communities.

The Philadelphia Model

With Philadelphia’s senior population continuing to grow, the demand for specialized behavioral health care tailored to their unique challenges has never been greater. CBH developed the Philadelphia Model, a groundbreaking model that combines behavioral and physical health services within nursing homes.
